Students Living Within Our School District:

Please pick up an enrollment packet from the school office at 904 S. Maple Road, Calumet, OK 73014. We will also need a copy of your child's birth certificate and immunizations, along with proof of residency (utility bill in your name at your address).

Students Living Outside of our School District:

You will need to apply for a transfer (form below). We take students as we have openings. You can see grade availability here. Please complete the transfer form and turn it in to the school office at 904 S. Maple Road, Calumet, OK 73014.
